Relief Community Support Worker

Job Purpose

To assist in the delivery of individualized community support packages to a number of clients with intellectual, physical and or mental health disability some of whom may present with behaviors’ that challenge.  Supporting clients in their community and some in their own home, working towards optimizing their social, psychological, intellectual, domestic, community living and daily living functioning



Minimum Education & Skills required

  • QQI level 5 in Health & Social Care or equivalent.   
  • Experience of working 1:1 in the community   
  • Understanding and knowledge of intellectual, physical and or mental health disability.



    Job Duties & Responsibilities
  • To plan, develop, implement, monitor and review appropriate educational, recreational and occupational programs within the community. 
  • Liaise with the Programme Supervisor through regular meetings, reviews and daily contact to uphold the values of Social Role Valorisation.
  • Develop a community profile that reflects the individual’s interests and assets through researching the local area and making contacts to find valued roles for the individual.

     

    Additional requirements
  • Full Clean Driving Licence and access to a car for business purposes is required
  • To promote and be actively involved in teamwork at all times.
  • To liaise with families, the multidisciplinary team and other personnel as directed by the Community Services Manager
  • To maintain a basic knowledge of first aid/personal care and to implement good hygiene practices.
